Please email a high resolution (300 dpi) headshot and images of your artwork to callfa.dbc@gmail.com. It is okay to show images of one or two pieces of art on your website and on social media, but do not show more than two pieces.  You can show an unlimited number of images of the artwork/s, but we encourage you to be creative and show details and the artwork in progress (think of them as teasers).

You cannot save the form and return to it.  If you wish to contemplate your answers, we recommend you copy and paste (ctrl + v) your answers from a personal file you can save.
